By entering the Sweepstakes, entrants hereby submit to the jurisdiction and venue of the federal and state courts of the United States and waive the right to have disputes arising out of the subject matter hereof adjudicated in any other forum. Any entry information collected from the Sweepstakes shall be used only in a manner consistent with the consent given by entrants at the time of the entry, with these Official Rules and with the junkie LLC Privacy Policy, which can be accessed at http://www.justjunkie.com/contestrules.htm. WINNER SELECTION: : One (1) winner will be selected in a random drawing from among all eligible entries within thirty (30) days of the end of the Sweepstakes, by representatives of Junkie LLC, whose decisions are and shall be final and binding in all matters relating to this Sweepstakes. The prize will be awarded. Grand Prize winner will be notified by e-mail, certified mail and/or telephone within fifteen (15) days of drawing. Winner (or winner’s parent or legal guardian if winner is under 18) will be required to execute and return a notarized Affidavit of Eligibility/Release of Liability, an IRS W-9 form and where legal, a Publicity Release form within ten (10) days of date of issuance. If the prize is won by a minor, said minor’s parent or legal guardian will be required to execute all required documents on the minor’s behalf.. If a prize notification or prize is returned as undeliverable for any reason, the prize will be awarded to an alternate winner. Prize is not transferable, assignable, or redeemable for cash. No substitution for the prize by the winner will be allowed. If necessary, due to unavailability of the prize for any reason, a prize of equal or greater value will be awarded at the sole discretion of the Sponsors. All Federal, state and local tax liabilities are the responsibility of the winner. Acceptance of the prize constitutes permission (except where prohibited) for the Sponsors to use the winner’s name, likeness, biography, statements, and any other similar materials for marketing, promotional and publicity purposes without additional compensation or any limitation. Sponsors are not responsible for changes in schedule of any element of the Grand Prize. All taxes/fees and expenses are the responsibility of the winner, including all necessary government documentation. Prize conditions may be added or modified by Sponsors. Total ARV of all prizes to be awarded in this Sweepstakes is $19.95 and the odds of winning the prize are determined by the total number of eligible entries received. GENERAL: Entrants
release and waive any claims they may have against Releases for any and
all injuries, claims, damages, losses, costs, or expenses of any kind
(including without limitation attorney’s fees) resulting from
